Could a Traditional Chinese Herbal Formula Help Women with PCOS Get Pregnant?
Polycystic ovary syndrome (PCOS) is one of the most common hormonal conditions affecting women of reproductive age, and it is a leading cause of infertility. While conventional medications exist, researchers have been exploring whether traditional Chinese herbal medicine can offer additional support.
A new systematic review published in the Journal of Ovarian Research looked at a traditional Chinese herbal formula called Cangfu Daotan Decoction (CFDTT). This formula contains eight herbs, including Atractylodes, Cyperus, Pinellia, and Poria, and has been used for centuries in Traditional Chinese Medicine (TCM) to address a pattern known as phlegm-dampness obstruction, which TCM practitioners often associate with PCOS.
Researchers analyzed 19 clinical trials involving 2,181 women. They found that when CFDTT was combined with standard medical treatment, women experienced better outcomes than those receiving standard treatment alone. Specifically, improvements were seen in ovulation rates, pregnancy rates, ovarian volume, follicle development, and uterine lining thickness — all key factors in achieving a successful pregnancy.
What does this mean for patients? The results are encouraging, but it is important to stay realistic. The researchers noted that many of the studies had methodological weaknesses and that safety monitoring was inconsistently reported. The evidence quality was rated as low to very low, meaning more rigorous research is still needed before firm conclusions can be drawn.

This systematic review and meta-analysis evaluated 19 RCTs (n = 2,181) assessing Cangfu Daotan Decoction (CFDTT) — a classical phlegm-resolving formula — for PCOS-related infertility. When used as adjunctive therapy alongside conventional pharmacotherapy (e.g., clomiphene, letrozole, metformin), CFDTT demonstrated statistically significant improvements over pharmacotherapy alone across key reproductive outcomes: ovulation rate, clinical pregnancy rate, ovarian volume, dominant follicle count, and endometrial thickness. Only two trials directly compared CFDTT monotherapy against conventional treatment, showing comparable pregnancy rates with limited statistical power. GRADE evidence certainty was rated low to very low due to high heterogeneity, risk of bias, and methodological limitations. Notably, 73.7% of trials failed to report adverse events; however, available safety data suggest CFDTT does not increase adverse event risk and may reduce incidence of OHSS and LUFS. Organ toxicity cannot be excluded. Clinical takeaway: CFDTT shows adjunctive promise for PCOS-related infertility, but practitioners should await higher-quality RCTs before broad clinical adoption.
