Key Finding
Acupuncture combined with IVF significantly improved oocyte quality and embryonic developmental potential (p=0.011) in women with PCOS, while also reducing harmful gut bacteria and regulating glucose and lipid metabolism.
If you have polycystic ovary syndrome (PCOS) and are preparing for IVF, you may already know that PCOS affects far more than your hormones. A new study published in Frontiers in Microbiology explored how PCOS disrupts gut bacteria and metabolism — and whether adding acupuncture to IVF treatment could help improve your chances of success.
Researchers in China followed 60 women with PCOS who were undergoing IVF. Half received acupuncture alongside their IVF treatment, while the other half had IVF alone. Thirty healthy women were included for comparison. Scientists looked at gut bacteria, blood metabolism, and the fluid surrounding the eggs (called follicular fluid) to understand what was happening inside the body.
What they found was striking. Women with PCOS had notably different gut bacteria compared to healthy women — with higher levels of certain bacteria linked to inflammation and metabolic problems. These bacteria were connected to higher BMI, wider waist measurements, and imbalanced hormone levels.
The good news? Women who received acupuncture alongside IVF showed meaningful improvements. Acupuncture helped regulate blood sugar and fat metabolism, reduced the levels of harmful gut bacteria, and appeared to improve egg quality and embryo development potential — a result that was statistically significant.
The study also identified two specific substances in follicular fluid — methionine sulfoxide and boldione — that may play an important role in egg quality. Understanding these metabolites could open new doors for improving IVF outcomes in women with PCOS.
While this is an early-stage study, it suggests that acupuncture may work through the gut-hormone connection to support reproductive health in PCOS. For women navigating fertility challenges, this offers a promising complementary approach worth discussing with your care team.

This open-label RCT (n=90; 60 PCOS/IVF patients, 30 healthy controls) investigated gut microbiota dysbiosis, systemic metabolic profiles, and follicular fluid metabolomics in PCOS, and assessed acupuncture as an adjunct to IVF. Gut microbiota was characterized via 16S rRNA sequencing and metagenomics; follicular fluid was analyzed by untargeted metabolomics. PCOS patients demonstrated significant gut dysbiosis dominated by Lachnospiraceae, Blautia sp., and Escherichia-Shigella, correlating with BMI, anthropometric indices, and hormonal parameters. Acupuncture combined with IVF significantly regulated glucose and lipid metabolism, reduced Escherichia-Shigella abundance, and improved oocyte quality and embryonic developmental potential (p=0.011). Follicular fluid analysis identified methionine sulfoxide and boldione as candidate key metabolites influencing follicle quality. Clinical takeaway: Acupuncture may exert reproductive benefits in PCOS via modulation of the gut-metabolic-endocrine axis, offering a mechanistically grounded rationale for integrating acupuncture into IVF protocols for PCOS patients.
